This will happen if you use Windows 7 and deactivate Aero while running OBS. In this case OBS records whatever is visible in the area of the captured window. If something is overlaying/in front of your window, it is captured as well. Since you have OBS running in front of your window, OBS is capturing OBS as well.
Under Windows 10, or with Aero activated, this is different. In this case, OBS is able to grab the contents of the window without anything that might obscure it, because Aero provides every window with its own hidden frame buffer.
