Bug Report OBS Crashes

Hi, after not using OBS for a while I have come back and it just keeps crashing on me. Specifically when i try to add my webcam. Also when I tried to add window capture it kept saying that the window was minimised and I only seen a black screen.

I couldnt find the crash log that was all i could find, i followed the instructions but couldn't find a folder named AppData even when i searched in the C drive. At the moment I am trying again and everything seems to be working fine. So I am going to try and see if i can record this video and if it crashes again I will try my best again to find the files, but as it is working i don't want to risk closing it again.
 
Okay i found my crash dumps after a proper crash.

Now there was no crash dump for my earlier "crashes" as i think i used the wrong terminology, even time i added before the application just stopped responding and wouldn't close. It didn't actually crash like it just did.

Hi, when i'm trying to preview, record or stream my fallout 4 the game crashes. It doesn't matter if i'm in the start menu or in the game. I have tried every option I can think off (video options mostly), and I have searched the web for people with similar problems but I found nothing. I know this is not related to OBS crash but if you know a better thread then please tell.
