Bug Report OBS 0.16b Intel Quick Sync Video (QSV) Crash

Just downloaded the 0.16b update and I'm getting crashes with Intel Quick Sync Video enabled when trying to preview/record locally (haven't tried streaming to my Twitch though).

This crash only started occurring after the 0.16b update.

Let me know if you need any more information. :D

The update contains an old QSVHelper.exe for some reason, an update will be available in a few minutes
