Recently I've run into this problem twice. In the middle of streaming to Twitch, there was a disconnect and the reconnecting dialog appeared. Eventually, the stream reconnected. Upon resuming live stream, my viewers informed me that my mic was muted.
Post-reconnect, the Microphone volume slider was 100% red/active but no input was bumping the meter. Only after clicking to turn the slider grey/inactive and back to red/active did my mic resume outputting to stream.
As this was not a crash bug I don't have a log. Hopefully my description is enough.
